Caremerge Raises $2.1 Million for Mobile Long Term Care Platform

Chicago-based Caremerge announces a $2.1 million Series A round led by Grazyna Kulczyk, an entrepreneur and real estate tycoon based in Poland. Caremerge is building cloud-based solutions to help physicians, nurses, and family members provide better and more coordinated care to residents of long-term care facilities.

The platform is designed to help organize communication among clinicians, administrators, and family members. Clinical notes can be documented, distributed, and tracked. Family discussions can also be recorded and tracked. The platform tracks which activities residents have signed up for, sends them reminders, and e-mails family members and clinicians with a weekly summary report of what their relative has been signing up for.

Caremerge was founded in 2010 by CEO Asif Kahn after an eight-year career at GE Healthcare IT. Caremerge was selected to participate in a three-year entrepreneurship program co-sponsored by GE and StartUp Health.  It has also been accepted to participate in InnovateLTC’s accelerator program, which focuses on senior health startups.
